To be admitted to Lviv National University of Veterinary Medicine and Biotechnologies named after S.Z. Gzhytskyi a foreigner files the following documents:

1) Application form;

2) the original and the copy of the previous education;

3) the original and the copy of the document (annex to the certificate of education), which contains information of his academic progress;

4) a copy of the birth certificate;

5) medical health certificate, verified by an official health authority organ of the country from which the foreigner has come and which was issued no later than two months before leaving for studying in Ukraine;

6) a copy of the foreigner’s passport or other document identifying the person without the citizenship;

7) a valid health insurance (except for foreigners who have come from the countries with which the agreements on free emergency medical care were made);

8) 6 colour photographs 60x40 mm;

9) the copy of the certificate of foreign Ukrainian (if any)

The documents which are defined in subparagraphs: ( 2- 4 ) of this paragraph must be translated into Ukrainian and notarized.

The documents must be also certified in the country of issue in a way that is officially used in that country for such authentication, and legalized by the appropriate foreign diplomatic institution of Ukraine, unless otherwise is not provided by international treaties of Ukraine.
